Our case manager:

• assesses all prospective participants
• assists the caregiver and participant through intake and admission processes
• assists care giver in obtaining available financial resources
• assures required documentation
• assists with transportation arrangements
• completes discharge and follow up of participants
• manages the Continuing Quality Improvement System for participants’ charts
• gives in-service sessions to staff

The nursing department provides:

• hands-on physical care in therapeutic interventions
• appropriate nutrition
• psychosocial care
• communication with doctors and caregivers
• podiatry care
• flu shots
• glaucoma screening
• CPR training sessions
• in-service sessions to staff for better care of the participants

• assesses each individual's past and present interests
• plans and implements a varied and creative program that caters to the participants' interests, abilities and needs
• prepares a continental breakfast and assists in serving the noon time meal
• assists participants with the activities of daily living
• participates in care-planning as members of the interdisciplinary team
• provide continuous supervision of all participants
